Nurse Delegation Services

Nurse delegation in Washington State began in 1996, allowing caregivers to perform certain tasks such as administering oral, nasal, topical, and rectal medications, instill eye drops and ear drops, irrigate suprapubic catheter, perform oral and tracheal suctioning, blood sugar monitoring, and insulin administration. With the provision of the laws,  RCW 18.79.260  and regulations,  WAC 246-840-010,  written by the nursing commission, these tasks, normally performed by a licensed nurse, can now be delegated to caregivers.

The nurse delegator must teach and supervise caregivers and provides a nursing assessment of the patient’s condition.

Sound Mind Sound Body offers nurse delegation services to the aging and developmentally disabled population in a community health care setting. We currently serve those in Tacoma, Pierce, Thurston, South King, and Kitsap counties and accept state or private pay residents.

Before the resident’s admission to an Adult Family Home, Boarding Home, or Assisted Living facility, an assessment is done – this determines if the facility can provide the resident’s care and services. For DSHS clients, it is done by the Social Worker or a Case Manager assigned to the resident. Private pay residents and their representatives must have a  Nurse Assessor conduct an assessment at their own expense.

It is a visual “head to toe” inspection/interview with the patient and their representative, a 25-27 pages that includes the patient’s demographic data, history, medications, interpersonal and psychosocial behaviors, daily routines, likes/preferences and dislikes, activity assistance need and multidisciplinary need- PT/OT, skincare, home health, etc.

It is done every time the resident has a big change in medical condition.

Assessment is valid for one (1) year; a re-assessment is then needed and conducted by a Social Worker or Case Manager assigned to the resident or a Nurse Assessor for private residents.

Adult family homes, boarding homes, and assisted living facility providers must have a preliminary negotiated care plan prior to resident’s transfer to their facility, and a comprehensive care plan 30 days after resident’s admission to their facility.

It is a written plan of care specific to the individual resident’s needs.  It can be collaborated by the resident’s representative and the provider and signed by both parties.  It is then reviewed by caregivers and implemented. It is a continuous process of updates and revisions depending on the resident’s daily routines.

State Pay Information

State pay residents receive services from Washington State under Medicaid or DSHS. Each resident has an assigned Social Worker or Case Manager who in turn contacts the Registered Nurse Delegator and starts payment through ProviderOne (IPOne).
