Description
Before the resident’s admission to an Adult Family Home, Boarding Home, or Assisted Living facility, an assessment is done – this determines if the facility can provide the resident’s care and services. For DSHS clients, it is done by the Social Worker or a Case Manager assigned to the resident. Private pay residents and their representatives must have a Nurse Assessor conduct an assessment at their own expense.
It is a visual “head to toe” inspection/interview with the patient and their representative, a 25-27 pages that includes the patient’s demographic data, history, medications, interpersonal and psychosocial behaviors, daily routines, likes/preferences and dislikes, activity assistance need and multidisciplinary need- PT/OT, skincare, home health, etc.
It is done every time the resident has a big change in medical condition.
