Description
Adult family homes, boarding homes, and assisted living facility providers must have a preliminary negotiated care plan prior to resident’s transfer to their facility, and a comprehensive care plan 30 days after resident’s admission to their facility.
It is a written plan of care specific to the individual resident’s needs. It can be collaborated by the resident’s representative and the provider and signed by both parties. It is then reviewed by caregivers and implemented. It is a continuous process of updates and revisions depending on the resident’s daily routines.
